3) Fax Orders : If you prefer to place an order by fax, please print out the FAX Order Form using your browser's print function. Fill out the forms, add your signature, and fax them to us toll free at

Deliveries outside the continental United States (including Alaska and Hawaii) are shipped via US Mail. Additional charges may apply depending on total weight of your order. We will contact you in advance regarding additional charges.
